Enrolling Students

To register a new or returning K-12 student:

  1. Contact the Child Accounting Office at (570) 925-0932 to schedule an appointment to register your child.

     

  2. Download and complete the registration paperwork.

             *These forms will be available at your appointment, if you are unable to print at home.

    Parental Registration Statement

  3. Bring the completed forms and the following documentation to your scheduled appointment:
  •  Proof of student’s age (birth certificate)
  •  Student’s immunizations records
  •  Valid proof of parent/guardian’s photo ID    
  •  Custody papers, if applicable   
  •  Two (2) proofs of residence, showing current district address, from the list below:
    • Deed
    • Agreement of Sale
    • Lease containing parent/guardian name and verified by the building superintendent
    • Real estate tax bill or paid real estate tax receipt
    • Homeowner’s/Tenant’s insurance statement
    • Valid PA Dept. of Transportation license or identification
    • Vehicle Registration
    • Voter registration card noting township or borough
    • W-2 statement/IRS statement or tax return
    • Check stub from wages, public assistance, or social security
    • Copy of State/Federal Program Enrollment
    • Current utility bill (gas, electric, water or sewer only)
    • Current credit card bill

           

     If transferring from another school, it is helpful to bring:

  • Student’s last report card
  • Copy of student’s IEP and ER/RR, if applicable for Special Education

 

All registrations are processed at the District Office, located at

600 Green Acres Road, Benton (next to L.R. Appleman Elementary).
